As the title says, what is required to open a full service pharmacy in thailand?

You might get generate some useful advice from some of the old hands here if you provide a bit more detail:

1. do you mean establishing a pharmacy as a Thai or as a foreigner?

2. do you want to know about those issues or regulations that ONLY apply to pharmacies, or do you want to know about all those factors/regulations/etc that apply to ALL businesses AS WELL AS those that apply to pharmacies

You'll need a licensed pharmacist.

Pharmacy would be opened by a thai national and I am interested in regulations applying only to pharmacies.

full service pharmacies with prescription drugs are allowed in hospitals only.

A Farang cannot work as Pharmacist in Thailand. Your Co Ltd can open a Pharmacy but you need a licensed Thai Pharmacist to be on duty during open hours. The Pharmacy needs to get a license to operate they can apply at the Sor Sor Jor a.k.a The Health Dept at least that's what's required in Phuket it may vary from province to province. Some drugs cannot be sold by Pharmacies only FDA approved Pharmacies and they are mostly hospital based.
