No they do not become experts what they become is burnt out and quit. In 2009, I spent 7 hours in a hospital emergency waiting room with so much pain that I had to lie on the floor. In Thailand, I steer clear of the government hospitals, as they are during clinic hours and ER, which are separate, totally overcrowded. The challenge is that in most countries there is a shortage of doctors, and GPs are among the most in short supply, along with trained ER staff. The reason for GPs, at least in Canada, is the amount of BS paperwork and regulations that they have to go through, along with the start-up and staffing issues and costs. Trained ER doctors are few and far between because as trainees in the ER they burn out and can't wait to get out of the ER. Here the challenge is strictly money. The government hospitals are on the 30 baht system and are not making enough money to be able to keep their staff let alone hire more. We have all read lately about hospitals that are shutting down because they can not get the funds from the government.
