Yes, all you need is the 90 day report form and your passport (with any attached old 90 day form and attached departure card from your last entry into Thailand).

Just go to Immagration with passport, they sort it out for you, I find the new office here fast, easy, and they treat you good

The number from the departure card is what you need. Some call it an arrival card because that's when they staple it in your passport (upon arrival into Thailand) but it is a departure card to be used on your next departure.
