Missing 90 Day Report Stamp?

I have a non-immigrant O visa with a 1 year extension stamp that started June 16, 2011. I think they accidentally left the old "receipt of notification" in there because it is from the previous 90 days reporting from before I applied for the next extension.

I am leaving the country in August before the 90 days is completed. It seems odd that the old receipt of notification was left in there though. I told them the date I was leaving and got a single re-entry permit the same day my new extension began June 16. Perhaps that is why they did not give a 90 day return date because they knew I would be leaving. Is that ok or should I run back and check with immigration before going to the airport next week?

The new extension and single re-entry stamps were both stamped and issued on June 16th so they know I reported. I just don't want problems if they needed to include the "receipt of notification" they usually staple in.

Ninety day reporting is not reset when you get your second or subsequent extension of stay. The 90 day report stapled in your passport is still valid.

As said TM.7 only counts as a 90 day address report the first time. Your last TM.47 report is your last report on record and a new report was due on the date stamped on the receipt you have in your passport. So if that date has not passed and you will be out of country just start new 90 day count when you return. If it has passed and you did not report you may face a fine of 2,000 baht for late reporting if it is noticed (probably when you make your next report).

Well, I don't understand why the old receipt of notification is still stapled in there. It says I was supposed to notify by February 21 which I did on February 16 as a stamp says. Then I was supposed to be back by May 16 which I did on May 12 which was also the day I applied for a new extension and was given an "under consideration stamp." Then the new extension was approved and stamped on June 16. That's where I'm confused. Why is there no stamp anywhere saying the next 90 day stamp sometime in September?

I called immigration and they say it's no problem so I guess I won't worry.

Did you fill out a TM.47 on the May 12 visit? This is your first mention of a May visit.

Ninety day notifications, bottom part of TM.47, are usually stapled in your passport, not stamped. The OP could conceivably be long overdue for his 90 day address notification, since he was due to report his address before 21 February but does not have the receipt.
