Letter from my wife requesting visa for me ( One in Thai, one in English)

This letter must be printed out or handwrite?

Bankbook copy enough or need to take extra paper from bank?

It doesn't matter. Normally a signed and dated copy of the wife's ID-card is enough to show the relationship is still ongoing. Best to be safe and take the original bankbook with you also. (For KL the money doesn't have to be in Thailand).

The letter is just something extra, in case they have doubts it might convince them to issue a visa. Printed and signed will be fine.

Am sure written and signed would also be fine if you do not have printer available - I was asked for this 15 years ago in Penang and all they wanted was a paper for file. But as said it is not normally even asked - but do have her ID card and home register copies.
