Mario2008
Do you have a work permit? If so, there is no need for a residence certificate. Just take your WP with you.

Mario2008
You can download the form from the immigraiton website (print double sided if it is more than 1 page). Some immigraiton offices demand that you do 90 day reports. Alternatively you can get a residency letter from your embassy. http://www.immigration.go.th/nov2004/download/res_cert.doc http://www.immigration.go.th/nov2004/download/res_persondata.doc
mstribling
The immigration at Pang Nong Ron require two passport photo's. If you lie in your wife's home and have her TB easy. The first time I did it, I was living in a rental, and my landlady had to register me first.
