My friend has an American passport and has just extended his permission to stay by 7 days at Chonburi Imigration Office

His flight ticket home is on the 18th May and his permission of stay is only till the 14th May (4 days overstay)

What options are open to him

Why did he extend 7 days if knowing it was not long enough? He had the option for a border crossing for a new 15 day entry if from most countries so that would likely have cost about the same from most locations. That option is still open. He could overstay but risks jail if caught by police (not likely for most but could happen) and overstay fine of 2,000 baht on departure. Or change flight.

Yes your right, but it his first time in Thailand and went to the imigration office to seek advice. It wasn't till he got back and checked his passport and found he had only got 7 days prmission to stay.
