On 18 February 2012, the moon was waning. I am not sure if that part of the form needs to get filled in but if it does, the district (amphoe) official will put a tick mark next to "waning moon"

The "signature of notifying person" is the signature of the person who notifies the birth to the amphoe office, which means that the corresponding circle gets ticked.

In the space for "Signature of Registrar Acknowledging Birth", the amphoe official entering the birth in the official records will put his signature. You don't have to fill in that part of the form.
