I'm guessing that the health certificate form is the same if used for a work permit or a drivers licence, but can anyone verify that different forms are not needed for different purposes?

Used the same one for both items you mentioned before with no problems.

Don't believe the gram negative syphilis test is required for the Thai driving license test - just the basic clinic health check i.e. BP, basic exam and questions about general health.
