Looking for Biometric application form pls - UK

Hi. I am having trouble finding the right biometric application form on the UKBA website.

my wife has never had her biometrics done before

she is in the UK, on a 2 year sett visa.

I will be applying for Ilr visa soon, and require an application form for this.

can someone pls point me to the web page

If she applies by post or online they will send her the form when they receive her ILR application.

If she applies in person they will do it when she attends for her appointment.

we will be using the premium service for the iLR visa

I know they will take my wifes biometrics the same day, but thought there was an application form I could fill out.

edit or do they send the app when you request an appointment online for ilr?

How do you enrol your biometric information and apply for a biometric residence permit?

If you make your application to extend your stay in the UK by post or online, we will send you a biometric notification letter after we have received your application. You should then enrol your biometric information at one of the post offices offering this service across the UK. For more information, see the Enrolling your biometric information - postal applications section.

If you make your application in person at one of our public enquiry offices or using our super premium service, you will enrol your biometric information on the same day. For more information, see the Public enquiry offices and Super premium service sections.
