Can anyone tell me what the next strength of pain killers available in Thailand is after paracetamol and ibufrofen/neurofen?

I don't want to end up getting something too powerful but would like something with more strength than the above.

Its for headaches/sun stroke.

Co-codimol probably - various strengths available - start at 8mg codeine plus 500mg paracetamol and go up to 60/1000 (prescription strength).

Co-proxamol possibly too - though care must be taken as its an opioid (so is codeine - but codeine is no real worry taken by mouth as it metabolises less easily) - also avoid if alcoholic.

Codeine is a controlled substance in Thailand (class II, same as opium and cocaine).

Will not be available OTC, it might even be hard to find a hospital willing to prescribe.

I guess the strongest otc painkiller will be tramadol. Strangely enough controlled in many countries but not Thailand.

Often taken together with paracetamol as they work very well together.

Will not be available OTC, it might even be hard to find a hospital willing to prescribe.

I guess the strongest otc painkiller will be tramadol. Strangely enough controlled in many countries but not Thailand.

Often taken together with paracetamol as they work very well together.

I bough co-codemol from the local chemist (its closed down now) - that was about 3 months ago. There is very little codine in it. Most chemists will also write a prescription for you too - otherwise, any clinic will if explained (especially for something so weak as co-codemol OTC)

And as with al opiates, be careful.

Tramadol has one annoying thing and that is that it takes a while to kick in. Sometimes up to 2 hours!

So don't go popping extra pills when you think they're not working. Just be patient.

One 50 mg tramadol capsule along with one 500 mg paracetamol is already a pretty powerful combo considering it's low dose.

Safe combo as well, ultracet brand (haven't seen yet in Thailand) is 325 mg paracetamol and 37.5 mg tramadol...

Biggest side effect with many people is nausea, and as always with opiates, better not drive or operate machinery...

10 large bottles of Chang Beer.

Do not take Chang Beer with any pain killers however, as alcohol should not be mixed with most medications.

Strangely enough though, if given the choice, I would be tempted to try the big bottles of Chang rather than any drug medications.

I get very fast relief from most pain, even severe dental pain if I drink enough of it, and almost no otc meds will do this for me.

Then I also find that it is a very good diuretic.

But I cannot take this at work, although I definitely do take it, and have taken it if I have pain from tooth extraction on a Friday, which is why I often schedule all my dental procedures on a Friday. It sounds not PC to you in America, perhaps. But it is a good remedy, and a long time remedy, and it works for pain.

I think it is also safer than toying with opioids for short term acute pain episodes.

So this is what I take if I can.

Each to his own. And for me, good for the pain.

I have often taken this combo with good effect, the paracetamol takes the edge off before the Tramadol kicks in.

for headaches caused by heat/sunstroke i would suggest you hydrate well, drink electolytes and buy a hat.

treating this kind of headache with ever strengthening painkiller, especially opiates cannot be good

Not sure what the local chemists sell, but legally any medication containing codeine should only be available at hospital pharmacies and on prescription, regardless of the dose.

The headache resulting from sunstroke is associated with dehydration.

The treatment for "sunstroke" is cooling and rehydration.

Powerful analgesics have no part to play. Paracetamol should be adequate if taken on a regular basis.

The Rehydration process will be aided by using "rehydration salts/ powder" obtainable cheaply from any pharmacy.

If a full recovery is not achieved within 24 hours medical help should be sought.

Never attempt to self treat a child who is thought to be suffering "sunstroke" ALWAYS seek medical assistance.

Prevention of course is better than cure !

Stay out of the sun, avoid getting overheated and always drink plenty of (non alcoholic!) fluid.

As others have said, if the headache is in fact due to "sun stroke" or anything close to it, treatment is rehydration and cooling and nothing stronger than paracetemol should be taken. Be sure to rehydrate with fluids that contain electrolytes as sometimes the headaches that come with dehydration are related to electrolyte imbalances. In addition to sports drinks and rehydration salts, green coconut water is excellent (and tastes better).

Thai nam minao and nam som ( lemon/lime juice and orange juice) freshly made are also good as the traditional Thai way is to add a bit of salt and sugar to it, and the citrus contains potassium already. (You get this at any of those stands with fresh fruit and blender).

But absolutely tops in my experience is green coconut juice. It's as effective as IV rehydration.

Keep rehydrating until you are urinating a good volume of clear colored urine. As long as your urine is at all dark, you are on the dry side.

And if you have headache or otherwise feel ill be sure to replenish electrolytes as per above. You can get hydrated OK with just water but your electrolytes may still be out of kilter and this will cause headache and other symptoms.

They are tension headaches that occur on a regular basis but also when in direct sunlight for short periods or when body temperature rises slightly. So I called it sun/heat stroke but I guess that isn't really what sun stroke is (?).

Its nothing to do with dehydration either.

Tamadrol sounds like a good choice. Do I just ask for that or does it have a brand name in Thailand?

Tramadal is the generic name and there are many brands in Thailand. Any large pharmacy will have it.

Do not mix with alcohol and do not drive after taking it. Effects can be prolonged, more so than with say codeine, you may find that even the next day you are still a biot loopy.

Some people have strange reactions to it including psychological disturbances.

That said, it is an effective pain killer and certainly stronger than paracetemol and NSAIDs. Taking it with paracetemol wil augment its effect.

I trust you have been evaluated for these headaches and more serious causes ruled out?

Was watching TV the other day (UK TV via internet) and a doctor was advising that coconut water is very good for pregnant women (seemed it is often advised these days) - just a side note.
