I had a computer crash a couple of months ago and have lost previously saved Tm 7/Tm 8 details and unfortunately forget how some of the details shall be filled at both the application for extension of stay and the re-entry application.

First question I have is on the application for extension regarding "kind of visa" Shall I there write the Visa I had and the Visa the extension is based on or only write retirement and last arrival in the Kingdom?

Second and third question is on the application for re-entry permit. Is it necessary to fill in "I intend to go to" or can I let it stands open? Third question is regarding "My previous visa for Thailand is of the category of" Do I here write Non Immigrant O and where it was issued and then first arrival in Thailand with that visa and under "permitted to stay up to" write the date which the last extension expire?

1. At visa you write the kind of visa you had: norally a non-immigrant O visa

2. You can leave it open

3. Non-immigrant -O, date current permisison to stay ends.

(Get the extension first and than fill in the application for the re-entry permit, with the new permission to stay.

You do, indeed, need to queue up again for the re-entry permit at larger offices (Chaengwattana, Jomtien, Chiang Mai, etc). However some smaller offices do allow you to apply for both at the same time - certainly Maptaput (Rayong) did so in my case last time round. If this is also possible at your office, you'll need to provide a second set of passport copies for the re-entry permit.
