Apart from the TM7 and the "Acknowledgement of Penalties for Visa Overstay", Phuket Immigration have now produced a third form which has no number and no title in English and is printed on the back of scrap paper.

A friend of mine wasn't sure what he was supposed to write and he was told to come back with his wife! Don't know if the immigration guy was having a bad hair day, but I didn't think that was very helpful.

 

Anyone else seen this form and know what to put in the spaces?

Anyone able to translate what it says at the top?

Looks like additional contact info maybe for police or other gvt. org.  so they can see if  property owners are reporting the presence of an alien.  Of course if you are retired the "company name" is N/A.

 

Aren't all the check boxes districts in Phuket?

It looks similar to the report my landlord supposes to file for my husband when we made a 90 days report for him. (We moved from Bangkok to Chiangmai)

The check box are names of district and sub district in Phuket.
