I'm getting there on a 60 day tourist visa next week.

I visited once before 2012 and have an unused multiple entry 180 day visa on my passport from last year. Will this unused visa pose any problems/questions?

And this time I want to stay indefinitely. How many visa runs can I, as an Indian passport holder, expect to realistically be able to do?

Your unused expired visa will not be a problem. But getting another visa nearby could be a problem because some embassies and consulates will not do a visa for an Indian citizen.

As far as I know the Thai Elite program is has no restrictions based upon citizenship. See: http://www.thailandelite.com/index.php

The Jack Golf company seems to do this for Indians as they list a specific price for Indians.

i think Jack golf only did border runs when they were in business.

Vientiane has not been doing them some time now. I am sure Savannakhet is the same. See: http://vientiane.thaiembassy.org/docs/Restricted%20Countries%202013.pdf

Kl or Penang may do them. You should contact the them to see what their policy is.
