Katoom Posted August 31, 2014 Share Posted August 31, 2014 Hello All, I have tried a number of different methods but cannot seem to get hold of a copy of the captioned application form in a correctly formatted file; it just seems to be all over the place. Can I ask if any member has a copy where the formatting is at least intelligable enough to be able to fill out neatly please? Or is that just the way it is?
