You need one photo to attach to the TM8 application form. http://www.immigration.go.th/nov2004/download/pdf/tm8.pdf

There are spaces on the form for departure and return dates but they are not important because they will not be on the the re-entry permit. Some offices will let you leave them blank.

You will need copies of your passport photo page. visa. entry/permit to stay stamp, extension stamp and TM6 departure card.

Single re-entry permit costs 1000 baht and a multiple 3800 baht.
