In February I had a little stroke about not 1 minute from then one my right leg don't want to work almost no more, I have seen a neurologist went to him a few months with NO result, about 1200 Bhat for not 5 minutes consult.

I go regularly see my hart specialist, nice doctor I see him about every 3 months for my medicines.

Now my questing is what kind of doctor do I need to consult, I am confused whit all those titles they have now?

With a stroke that has already happened, the only things that can be done is (1) preventive measures against future strokes and (2) physical therapy to help improve function. Unfortunately the quality of PT services in Thailand is poor.

Given that you are on HAART, the factors that led to the stroke may be related to it; HAART is known to increase the risk of stroke due to its metabolic effects. This puts you in a different situation in terms of prevention than the average stroke patient and I would suggest you see your HAART physician regarding measures to prevent recurrence, as these need to be instituted with full consideration of your HAART regiment. If your HAART physician is not sufficiently up to date on this sort of thing, I highly recommend Dr. Asda at Bumrungrad in Bangkok.

As mentioned quality and aggressiveness of PT services in Thailand is generally weak so it may take some trial and error to find a physical therapist who is effective, but it is worth the effort. The first 6 months after a stroke are key, beyond that it becomes hard to improve function,
