any help where i can get this form to download it. i did look on immigration forms and did not see it.

That is the Phuket only form. You must look on the phuket immirgaiton website and see if they have it there for download.

to be used for retirment visa extention/renewal

It is the "Phuket" form which as far as I know is not available for download.

The form is used to register the address at which you stay.

From personal experience I can affirm that Phuket Immigration will also accept the TM30 form

The registration must be done prior to applying for an extension of stay based on retirement (there is no retirement visa! )

Today (March 2), I showed up at Phuket immigration with a properly completed TM30 form.

The Immigration Volunteer told me that they will no longer accept the TM30 - you must use the Phuket-specific form, which is NOT available for download. They have plenty of practically illegible copies available at the information desk on the 1st floor. You complete one there, and when your queue number comes up, the officer enters your data, prints a legible copy for their records, and gives you the bottom portion of that newly printed form as your proof of filing..

I was told exactly the same.Took the "phuket" form and told the volunteer I would complete it. Make sure you get a queue ticket ! Then ignore the "phuket" form and present the TM30 to the Immigration Officer who will accept it !
