Replacing stolen licence in Phuket or Bangkok

Hi All,

I live in Bangkok but am now in Phuket. My motorbike licence was stolen. I have the police report. Does anyone know:

1. Can I replace the licence in Phuket or do I have to do it in Bangkok where I live?

2. If I can do it in Phuket where do i go and how long will it take?

3. In either case, do I need to bring anything besides the police report and my passport?
