Does a flight have to be booked before applying for a re-entry permit?

No it does not.

You can also get it at the airport when leaving.

Thanks. It's just that on the form it asks for the departure date of the flight.

Those dates mean nothing and some immigration offices let you leave them blank. You can put any dates you want to in the spaces as long as the return is before your current permit to stay date. They are not on the re-entry permit stamp.
