1. & 3. See under QUALIFICATIONS OF AN ALIEN ENTITLED TO APPLY FOR A WORK PERMIT. Work permits are not available for tourist visa exemption or tourist visa holders.

Your employer needs to submit the application. You only need to provide a copy of your passport( signed) and your resume( CV)

Yes it can be applied with you in (with a visitor transit stamp ) or outside Thailand and you would need a copy of your passport and resume ( CV) Your employer would be the one submitting the paperwork along with the company supporting documents.
