... I actually get a form entitled " Application for Substitute of Certificate of Residence "

I don't have a Certificate of Residency yet and I need one for banking purposes.

Will the downloaded, incorrectly titled application form get me what I need?

There is no numbered form to apply for a certificate of residency. Those are for those on permanent residency to get a residence certificate.

Having got the form printed out and completed, what supporting paperwork is needed? Obviously copy passport, copy arrival card, and I was thinking of a copy of the apartment rental and receipts back for a year. Is that enough? What is the fee, and the length of time to process it?

What is needed depends upon the office you apply at.

A rental agreement at the least will be needed.
