April 22, 201610 yr You apparently have an extension of stay based upon retirement not a visa. You will submit a TM8 form with a 4 X 6 cm photo attached, Download: http://www.immigration.go.th/nov2004/download/pdf/tm8.pdf You will need a copy of your passport photo page, visa, entry/permit to stay stamp, extension of stay stamp and TM6 departure card. The fee for a single re-entry permit is 1000 baht and a multiple is 3800 baht.
