I am applying for the above in OZ.

The form from the consulate says I need a doctors submission form that is available on the Thai Immigration Website.

I have tried but can't find this to download???

Secondly is a Stat Dec re 75,000 Baht per month income OK?

For income it states that a income certificate is needed. I don't think a statutory declaration will be accepted. Perhaps something from your pension provider(s).

There is no retirement visa. Only the OA.

OP if you Google " medical certificate for Thai visa" you will see a PDF to download first hit.

You need tick all boxes as per au Thai website. You can apply via mail to Canberra. Note original and copies need to be bound and notarized. Website states a JP is sufficient but I would ring and check. They were starting to suggest JP not acceptable. With medical make sure the Dr or clinic puts a stamp on it. Some quacks will require blood tests others nothing. You need read web site in detail
