Which Form for Extension of Non-O (Multi-Entry)

Hello,

I will be going to immigration to extend my non-o multi-entry for 60 days tomorrow AM. I'm hoping to fill the form out today so that I can avoid some of the lines tomorrow. Can someone confirm that the form I need is TM-7 on the following site?

This form looks different than the form I filled out in the past, granted I only extended a tourist visa in the past. If this is not the correct form, can someone link me to the correct form? Thanks!

The TM7 form is the one you need.

Excellent, the form on their site looked a bit different and being Thailand, I'm not sure if they updated the form or just didn't update their system with the newest version. Either way, this is the best I can find so... much appreciated :).

The form has not changed - print on front and back of A4 paper and include phone number on rear side.

You might see some differences if you were looking at the MS word version and did not have the correct program to open it.

I was using Open Office, it does look slightly different. I thought I recalled the picture section being int he top right rather than the back but I suppose I'm mistaken. I haven't needed to extend in roughly a year.
