Form of employment certification - Lao, Cambodia, Myanmar EP

My other half is Lao and working here on a two year EP. 

While doing a first 90 day reporting today she also got a multiple re-entry permit.  However when the permit was given she was given a  "Form of employment certification" which must be completed every time she leaves the country.  

What it appears to be is a document that has to be signed  by her employer to prove 1) she is  still working there, and 2) she has permission to  go home.    Apparently it applies to EP holders from Laos, Cambodia and Myanmar.   

Her employer is OK to do that but the level of detail requested is like a full rerun  of the EP application process .

Things like last year revenue, salary earned, tax paid, blood type, reason why job is not filled by Thai employee etc.                                                                           

Does anyone  have experience with this form ?

Is it a fill in name and address and get boss to sign rubber stamp job or does every field have to be completed .  Her work permit was processed by an agent at an extortionate fee and i'd prefer not to have to use him to validate the form against the original data if we want to travel.

But my biggest concern would be that we get the form completed and signed book some travel and then they reject it at the airport.

 

Any advice appreciated.
