90 day reporting first time advice required

I have to do my first 90 day report by 6th December. 

I am here on stay based on retirement which was granted to me in March

By coincidence both times I was due to report I was out of the country

I have a multiple re entry visa

I have lived at the same condo although I have travelled around Thailand on tourist trips

I will be using the Jomtien immigration office.

What documents will I have to provide? My passport? Proof of address- if so in what form? - what cost? My arrival card?  A TM something or other? If so I would be most grateful if somebody could tell what the document is rather than just it name. Thanks.

One small worry id that the IO stapled in two thin pink receipts in my passport which I removed - I seem to have lost them - will this be a problem?

Even the local knowledge about Jomtien immigration is more likely there.

 

You should not need anything more than your passport and a completed TM47 form to do the report.

The lost slips were probably the receipts for your extension and re-entry permit. No problem it you don't have them.
