Hello,I  a have a one year retirement visa(that's what it say's on the visa)I have to go home for a few weeks,what form do I need for permission to leave the Kingdom?

You don't need permission to leave the country, you need to buy an re-entry permit at immigration or at the airport before you leave so you keep your permission to stay alive.

If you have a visa.. You need only enter the kingdom before the expiry date of the visa. 

 

If you have miss informed us, and really you are on an extension of a permission of stay, then as above you need a re-entry permit, to keep the permission of stay valid, and to return prior to its expiry. 

 

Another thread where being clear on what basis you are in the kingom on is very important.

@splodger do you have a visa (obtained from a consulate outside Thailand) or an extension of stay (obtained from immigration in Thailand)?

 

If you have a visa is it multi-entry? Will you be returning before the "enter before" date? If both are "yes" you need do nothing.

 

If you have an extension of stay and will be returning before it expires then you need a re-entry permit, if the trip is a one-off then a single will do, 1000 Baht. Get it at immigration or at the airport on your way out.

I should be grateful if you kindly posted a scanned or photographed copy of your retirement visa so that I may add it to my collection of Thai visa stamps. Please pixelate or otherwise obscure any personal information on it (name, passport number, etc)
