30days at immigration after stamp of 60d at airport

Got METV which expired now after I entered Thailand. I got my 60d stamp at the airport and will have to go to immigration to get my 30d extension and pay 1900 bath.

Which file do i have to download and fill in before I go there? Where to find the file online? What else should I take with me when I go to the office. I'm thinking to go to Pattaya Jomtien immigration office.

Fill out the TM7 form, print on both sides.
