Hi All, my first marriage visa runs out on 16 August 2017.  When should I start applying for my 2nd marriage visa?  

Not sure what you have since there is no marriage visa.

f you have a multiple entry non-o visa based upon marriage issued by an embassy or consulate you do not need to get a new one until after you do the last entry on it.

If you have a one year extension of stay (it is not a visa) based upon marriage issued by immigration you can apply for a new one from now up to the last day if you want to.

Extensions based on Marriage can be applied for anytime during the final 30 days.

45 days at a lot of offices.

Going for my 3rd extension but the wife might not be able to go with me, is it necessary for her to be there for the extension, thanks

Yes it is mandatory your wife is with you.
