I gave them application form, passport & copy, passport photo, and confirmed flight into Thailand. NB. no outbound flight details given or even queried, but I was wearing a nice shirt with collar[emoji4] I arrived at 9:30 no queue at all. The only issue was one of the $20 had a tiny 2mm tear in it that I didn't notice.
