You are wrong. It was changed to two 30 day entries per calendar year in 2015. -
Unless you are doing the 90 day report where you did your previous report or extension of stay you will have to formally change your address to the hotel where you staying.

You appear to be in Ubonratchaithani. The immigration office in Phibun is no longer open. There is a office in Ubon city at Rajababhat University now. You cannot get a extension at a border crossing. -

Your first 90 day report will not be due until 90 days from the day you apply for the one year extension. -
Thirty days is the standard number of days. But if your extension isn't approved by the end the 30 days it can be longer.

If you enter the country with a re-entry permit or a multiple entry visa a new TM30 report is not required. Otherwise a new one is needed. -
You will use both passports when entering the country. Immigration doesn't require letter a letter when the passport is issued outside the country. They will transfer the stamps without it.
