It would depend on the office, also agents dont usually do marriage extensions

Not all offices require witnesses.

If needed they would be people that know you and your wife to confirm you are living together.

The pink is the original.

Some IO' request witnesses when applying or the attendance of the Poo Yai Baan.

Others like mine request witnesses present when they make the home visit.

 

For what it's worth, I've seen lemon coloured marriage certificates as well.

When doing the marriage extension with a letter from the Embassy.
Is a bank book required?
