Exists since a while, maybe your immigration office just started using it?

If you Google for "site:thaivisa.com stm2" you find a few threads about it.

I have been given the acknowledgment of terms and conditions for all for my extension applications (12 of them). And the one for overstays for a few years now.

It appears your office are doing the same thing as the office has has done for a few years now by creating a 2 sided form for both acknowledgements.  Actually the official STM 2 form is only for the conditions of extension to stay.
