It is up to the operator to look at the ramp, the water level, know his vehicle, his tires, the condition of the ramp, etc. and decide how far down to back his vehicle. Looks like he decided poorly. The photo, presumably taken some time around the time of the incident doesn't seem to show a lot of wave action.

I grew up using manual come alongs a hand winch etc. 1

Usually a 2 man job. 1
