Marriage Extension - Bank Letter

Good morning all.

 

Very quick question - could somebody please let me have a copy of a 'typical' bank letter used for a marriage extension?  Obviously with personal data blanked out.

 

Money in bank, seasoned, bank book up to date, just need to know what the letter should say.
