Sorry if this is a dumb question but it seems that both of these locations do things for visas, I recently did my 30 day extension at the former but then when I went back after losing my TM6 form to get a new one I was directed to go to the latter.

 

Id like to apply for a covid extension now so which place should I be going to for that?

If on a entry from a tourist visa you would apply at the Chaeng Wattana office.

If you entered visa exempt it done at the Muang Thong Thani office.

Also, is this TM30 form strictly required? The staff at the serviced apartment Im staying at have given me a print out with my name, nationality, passport number and DK number and appears to be from the immigration site so I guess they have done the reporting online? Is that enough?

That will be sufficient. 

Sometimes for various things immigration wants to see what you described.

They most likely will for a covid extension. 

Clearly your hotel filed a TM30 and you have screenshot of that.

No problem.
