If flying out of BKK or DMK they provide a 200b service to provide form , photo and basic copies of pp. 

So 1200b single or 4000b multi.

 

Doing it yourself print the TM8 back to back (one page.

You only  need one photo.

You should mention the land border crossing for specific advice re obtaining reentry permit at the particular crossing..
