Getting a TIN in Bangkok

Internet descriptions of the documents required to get a Tax Identification Number (TIN) for a foreign individual vary.  Does getting a TIN in Bangkok (specifically at the tax office in Khannayao district) require a residence certificate from Immigration? I really don't want to have to go to that ghastly place more often than I need to.

I got one in Bangkok. Passport, Visa, electricity bill, withholding tax certificate of some dividend payments (you can use the equivalent for interest payment of your bank.)

Bang Khun Thian district office- just showed Thai driver license, stamped mailing envelope from Immigration and passport.

Khon Kaen accepted Passport and Yellow Tabian Baan.
