Went to hospital#1 in Bangkok with a cellulitis/infected sebaceous cyst to remove.

They said I should go straight to inpatient surgery with general anesthetic and stay 3-4 days or longer depending on outcome. Cost for day#1 needed upfront 130,000THB deposit without insurance with added days extra albeit less, perhaps a grand total of 170,000THB.

I went away to think about it and get a second opinion at hospital#2 in neighboring district in Bangkok and was quoted 4,900THB with ten minutes work cutting it out using local anesthetic and to return each day for clean bandaging.

This kind of pisses me off, that doctors opinions and price differences can be so great.

I've just had a similar experience here in Phuket when I went to enquire as to the cost of a colonoscopy, mainly because I had the last one some seven or eight years ago, when it cost me 18,000 baht or thereabouts.

This time the administrator for the unit gave me two quotes, one for no polyp removal at 47,000 baht and a second quote in excess of 90,000 baht if there was a polyp removal involved – – I will seek a second opinion from other hospitals here, if indeed I decide to go ahead with it.

The more important issues to my mind, given that you say you have cellulitis are:

  1. Are you on antibiotics? Which ones and for how long? Is the infection resolving? If you have not already done so, circle the red margin with a felt tip pen and watch to see if it increases, if it does then IV antibiotics may be needed urgently.

  2. Are you sure both places planned immediate full excision of the cyst? As this usually cannot be done when there is infection present. Normal management for most infected sebaceous cysts is to perform an incision & drainage (I&D) to let the pus out (which can indeed be done pretty simply as outpatient under local anesthesia) followed by full course of antibiotics, then later once infection is fully resolved, a deeper excision to remove whole cyst and its capsule (necessary as it may otherwise recur -- but no great rush). This later full excision can usually also be done under a local anesthetic but it does depend on the size and depth of the cyst.

I suspect the second quote was not for full excision (which as mentioned would normally not be done right away anyhow) but rather just for I&D and a course of oral antibiotics. You would then still need a full excision later. Either that, or they did not think there was an infection at all and planned a simple excision. Just how bad is this infection? (Redness etc?)

The first quote is a bit puzzling but it rather sounds like they were proposing to do an immediate full excision probably followed by IV antibiotics for a few day, the latter would explain the 3 day hospital stay (and can indeed be necessary with cellulitis). . Why immediate excision rather than I&D and a course of antibiotics first, I don't know but there may have been a reason related to the severity of the infection and size/depth of the cyst.

In short I think this is not just a price difference but a significant difference in proposed medical management and there are serious considerations as to which is best that you need to look into. Cellulitis can be quite serious.

Shout it out! I want to trust hospitals and doctors and hate holding my wallet tight when I should be relaxing into the tender care of the experts.

I am happy to pay for my care, not to be fleeced.

You do have to watch doctors like a hawk, mainly unnecessary meds, even govt hospitals prices are up for foreigners, certainly one near me, best to shop around if you have time.

Recently i checked on the price of a knee meniscus op,

Govt 200k

Private 250k+

India top hospital 80k to 160k

2nd opinion, ask the doctor if he works at another hospital, the prices may be more affordable.

I think like mentioned, you need to factor in the issue, how serious it is, location, if you require frequent follow up visits, etc.

Definitely believe some hospitals are focused more on medical tourism so the prices may reflect that.

Similar in the US with noticeable price difference with hospitals only 5 or 10 minutes away.

I’ve posted about this here more than once: a detached retina. It happened to me about 18 years ago when I was still living in Phuket.

I went to the Bangkok/Phuket Hospital with symptoms; the ophthalmologist diagnosed a detached retina within five minutes. He couldn’t do anything there and advised me to go to the Bangkok Hospital in Bangkok, where they could perform laser eye surgery.

I couldn’t believe that the doctor could diagnose it so quickly, so I went to the International Hospital, also in Phuket, on the same day for a second opinion.

The doctor there couldn’t find anything and advised me to drop by his private clinic in the evening, as he had different equipment there, once again, he couldn’t find anything.

By then, I had informed my insurance company about the problem and we had arranged an appointment at the hospital in Bangkok; they had also booked tickets so that I could be treated there the following day – it was indeed a detached retina.

I mean, even with a second opinion, you can’t be absolutely sure.

Yes, using Augmentin antibiotics at hospital#1 plus ibuprofen that said I had cellulitis. I was given this day#1 from hospital#1 and they just put a bandage over the cyst and said come back day#2 for re-bandaging with plan to see doctor again on day#3 (that never happened). On day#2 I was told without even being looked at I would need to see a doctor when I thought I was just there for re-bandaging. That's when a new much older doctor came in and said I should go straight in inpatient general anesthetic after 8 hours of no food/water to have the cyst removed. I asked how much (160,000 THB++) and thats when I left for a second opinion.

At hospital#2 the doctor said it was infected sebaceous cyst who said he has seen plenty like this, not an issue and he will cut it out now inside 15 minutes with local anesthetic and I went home with cyst removed (about 2cm) with plans for daily check ups. Same antibiotics given, along with anti inflammatory and swelling meds for a week or two. So far so good.

Bangkok Hua Hin qu 450,000 Hua Hin govt. 10 minutes down the road was only 38,000 all in and that was with a private room with Air Con

infected cyst by itself is one thing, cellulitis is another.

If you had cellulitis, the surrounding skin would have been bright red color and warm/hot to the touch.

allergic reaction to some cashews I ate all that in and out under 3 hours .. was somewhere like 2200 Baht

sounds excessive.

Had half a kidney removed (cancer) open surgery....walked out 4 days later.

Sounds like the upgrade to my (social security) private hospital... tiss looking swank.

I had one of those a while back (sebaceous cyst). Quite painful.

Was just given antibiotics and it cleared up. Hasn't returned since.

Not much, flights were cheap pre Iran war, 2+ hour flight. They will also speak excellent English, unlike here

There is a surrounding skin that is reddish, not bright red, was warmish, but since cyst cut out the redness has not expanded or seems a problem at the moment and not hot feeling anymore. Next doctors appointment after he cut me open on Monday is on Saturday so I assume the meds take care of the redness and see what doctors says later. The doctors nurse who dresses the wound daily says its not an issue. I have no pain, only apparent soreness from the wound I guess. So far so good.

I am not really sure why this started, the doctor said bacteria entering a hair follicle on my waist probable. I thought maybe from a new shingles sore, as the initial sore felt like that in a new location on reflection, though I've been vaccinated for that already and don't see it much now, but doctors in both hospitals didn't think so, indeed in the expensive hospital#1 the doctor who wanted me under general anesthetic didn't know what shingles was despite speaking English and I was not aware of the Thai at the time.
