I am getting my nose done and it will be under general.

To be honest I don't need a upper bleph (I am still young for that) but I was curious about it. Especially lower bleph actually cause I have some hollow eyes (always had them).

My questions where to you so I can get things from the patients perspective.

Are you happy with your chin implant and in your case what kind of anesthesia did they use?

Wouldnt the numbing injections be painful lets say if you are doing a chin implant? I bet there will be a few shots and the canula will be bigger and goes deeper...

well. Mine was local and I was sedated and it did hurt initially from the injects but that was all. Very happy yes.

Trust me when sedated the injects will be initially ONLY painful but youd be so out of it anyways and it goes away. LOCAL is much quicker recovery than general.

Atleast your not going to be strapped in WIDE AWAKE with NO SEDATION and BLINDING WHITE LIGHTS IN YOUR EYES and smelling your flesh burn and feeling injects etc :o

Exactly what happened to my partner in his 2nd revision.

He had his nose and chin done together under general. But unlike local you cant just wait 30-40mins and walk out. Youll be groggy and down for hours.

Did your partner had the revision WIDE AWAKE with NO SEDATION under the hands of dr. somyos?

Also, what's the point of sedation if you feel the pain of the injection of the local anesthetic? Doesn't it defeat the purpose?

No that was with sorawuth. Somyos took a long time and insisted he should be under general.

Sedation from experience DOES HELP otherwise the pain would be even more. But dont worry about it. Youll be fine its nothing.

Sedation depending on whats used helps you calm down, relax and even fall a sleep. Otherwise like my partner youd feel everything and youd start freaking out and your heart would be racing like hel_l etc.

I have 3 weeks off. Will the incisions and swellig be obvious at 3 weeks? When can I go back without looking I had done surgery?

Decide. What are you going to have done?

At 3 weeks youll be fine. Lots of the swelling would have gone away within the first 2 weeks and some more too by week 3.

We were fine by week 2 thought by 1 month much better.

He does teach at Uni but hes a professor. Have you seen professors? They dont start early. And I remember talking to him he usually only sleeps 6hrs -5hrs a night. And he starts his uni at around 10am or something like that. His clinic visitations are from 6pm onwards but usually complicated surgeries he starts late after all the minor procedures like botox, aquamid etc

And Im sure Milo stayed there for a little while which is why the Doctor maybe could see him that much. When I went for surgery I only stayed OVERNIGHT. So he saw me in the morning and another doctor later. Then he saw me when it was time to take the plaster and stitches off but thats all probably because i didnt stay at that hotel and went travelling etc.

i quite confuse of wat u saying.. at 1st u said u choose preecha instead of somyos in another posting (Need To Correct A Standard Nose Job For Thai ) on 2007-10-22 22:53:51 as below;

Hi, read with interest about the procedure various doctors can do,

I have a question could someone please answer??

in the Augmentation process using silicone on the nose, how does the silicone stay in place, for say using a implant to Augment a bridge of a nose etc

does the node tissue grow and mould around the silicone etc

i am considering an implant but wary of slippage from the implant as my understanding was it simply rested on top of your exisiting bridge, surely it needs to be either sown in or tissue grows around the implant so it does not move etc

Well i got back thursday from Bangkok and i have to say im thrilled with my new nose. It looks so much prettier whilst still looking like me, i will try and post some pics soon. I have high praise for Dr Amorn and his wife who took an active part in the surgery and the hospital was fantastic. The only downside i would say is that i didnt feel that aftercare was properly explained to me, although ive had no issues so far. I also lost quite alot of blood during surgery but that wasnt anything to do with Dr Amorns work, i guess im just a bleeder! Although the communication wasnt great i have to say he knew exactly what he was doing and its ended up to look exactly as i wanted and i dont have too much swelling either! I was slightly concerned at first as Dr Amorn told me i wouldnt be having general anesthetic but i was heavily sedated and dont remember anything untill i woke up. So at moment all is good. will post photos asap its only been 2 weeks and already it looks great!!!!! :o

Great news apples! Would love to see your before and afters.

BTW, do you know the technical details of your surgery? (ie, grafts, etc)

Was it an open or closed surgery? How was your bruising?

I had a closed rhinoplasty, they removed the hump and refined the tip, it also needed breaking. I had a bit of bruising for the first week and still have a very small one although its been 2 weeks swellings not too bad though. Ive sent some photos but theyre not great as i didnt like having my picture taken before too much but ill try to send better ones. The most irritating part was the packing and stitches! let me know what you think....

Hi Sampam,

haven't done anything so far. I am still trying to get more info on what the reason for my swellings are but haven't found anything which sounds convincing. Two doctors said they would hesitate operate on it (German doctors). Dr. Somyos recommended botox because he diagnosed a hypertrophic lower eye lid muscle and laser for the wrinkly skin. He also could not explain what could have gone wrong or what the causes for the swellings are.

Dr. Preecha would be ready to operate on the lid after two months, but I haven't made a decision yet. He doesn't talk much. Though, I trust him most. He would suspend the lid laterally in order to prevent an ectropian. He said he expects an improvement but wants to be on the safe side ("do no harm") which I think is very good.

Recently I learnt about a surgeon in London, Dr. Jane Olver, who apparently did a very good lower eye lid job on a lady who had six (!) lower lid surgeries. I saw a video clip and photos - impressive. But I have no chance in the moment to consult her to get one more opinion. What a pity. Have you heard from her?

BTW, do you know who is very good in injecting Botox in Bkk. I think Dr. Nantapath is very good, but she is much more expensive than others. Do you know how Dr. Somyos's skills are with Botox?

I was to have it in Feb then I had this nasty cough and tested for mycoplasma. Took antibiotics and prednisolone (triggered asthma) and I am still on these meds (antibiotics, pred. inhalers) So just waiting it out. Hopfully I will improve so I can go ahead with the surgery. mycoplasma is nasty and doctor told me it could last for over a year! My plans are all screwed.

But I will go ahead and do it with Dr. Somyos. How long do you think I should wait after pred. if I am going under general anesthesia?

Am I the only one who things Dr. N. is overrated? I have been going to her for sometime but I DO NOT think she is GREAT. She is just an okay doctor and more of a business woman I have found. She does offer a wide selection of treatments and they are easy to get...

i agree. shes just more hype and overrated.

honestly many doctors are good in this. Somyos as an example does perfect botox and lots of other dermal fillers too and had botox and aquamid done completely happy.

Well it also depends on your overall state of health eg BP, hdl/ldl levels, physically active etc etc?

But if your the average person then id suggest waiting atleast 2 weeks once you have been given the all clear.

Well my thoughts are this. Keep in mind what the german doctors said but also try some alternatives.

Somyos as an example is offering you a solution that could work and is totally reversible on its own its only skin lasering and botox . safe effective and reversible on its own. Preecha is willing to do more intensive work but then again would that fix the issue or possible make it worse? with your choices i would try somyos approach first (even maybe use of temporary filler to see if that will help eg collagen) and if that doesnt work then preecha's method.

Yeah if i remember jane's speciality was opthamology and shes now a facial plastic surgeon specialising in the eye orbital socket area. There may be one of the in Bkk too.

Dont rush into anything and remember you can always try conservative safe treatments like somyos with fillers botox laser etc and if that doesnt work going to something more radical. last thing you want is more scar tissues and compounding problems.

Nantapath is over rated. Somyos does it all including dermal fillers. The department at yanhee does it fine and so does bumrungrad tool.

Tried Botox out for the lower lid but it did not have any effect at all, least of all an improvement. So, Dr. Somyos recommendations are for now out of the question. Also, laser and filler do not address the underlying cause of the swellings.

You mentioned that Dr. Somyos did fillers on you (aquamid) and botox, with good results. I will try him out last time. Have you had fillers over the malar bone. I once had them with Nantapath and it looked good. If you tell me Dr. Somyos is skilled in that, I will go to him next time.

With regard to an oculoplastic surgeon (like Jane), I haven't heard of anyone in Bangkok. Have you?

I agree with you that they dont treat the underlying cause but rather hide it better. If you want to treat it properly then yes you do need surgery and it will need to be very extensive ie in removing minute scar tissues small changes etc etc

The fillers lasers botox etc will help conceal and minimise but not remove the underlying cause. I do remember Bumrungrad and BangkokHospital both have oculoplastic surgeons. Oh and there was a doctor called pungpapong i think in BNH who had training even at harvard medical school and specialised in oculoplastic.

Go try them and specifically try him and other will qualified oculoplastic surgeons and get their opinions too.

These surgeons specialise in the eye and orbital socket area so you might as well get checked out by them too and find out what they think but be detailed about your past medical history surgery complications etc

Sampam, Is Aquamed a permanent filler? Can they be used for cheek and jaw angle augmentation? How about orbital rims? Wonder if it can be done at the same time as my rhino. Does it require anesthesia?

Yes adam it is a permanent filler. However it can be apparently reversed by suction using a large syringe eg 18g and by repeated washes and suction so youd need to know where its been injected and the pocket etc.

Yes they can be used for cheek and jaw angle etc and orbital rims but you should talk to somyos about it as he doesnt recommend it for everything as it has a small tendency to move sometimes specifically with gravity. As an example he definately recommened chin implant over filler even though filler would have cost more.

For under eyes etc id probably go with something long lasting but reversible just in case i dont like it like hyaluronic acid with a high concentration of say 25mg/ml and consistency of eg 3,000,000 it could last near to a year in some. whereas the standard cheap injects are only 20mg/ml and 0.7-1.5million dalton.

lower eye as upper eye many surgeons. I have listed the best already-read my other posts- so long as you havent done it before and no complications then its easy.

Preecha is excellent for lower eye including revision too.
