Which Form For 60 Day Visa Extension

As per title, have a 60 day visa, need to get the 30 day add on thing. can someone please tell me which form i need to download and print from the sticky above for this.

Go to the Immig Police offit, they will do it you just sign a few papers.

Do you have a map for the Chiang Mai immigration office and if my 60 day TR doesn't expire until Nov 9th can I still get the extension now? And will it become active when my current one expires or does it become active immediately and overlap my current visa?

I answered that in your other post and the Immigration Office is between the shopping mall and the airport entry I believe.
