Off to KL tomorrow and need some advice post haste. Firstly, do they give out the TV application form at the Embassy in KL, or do you need to download it and take it with you? If so, where do you get it from? Also, the requirement is for the photo to be 6x4 cms - I seem to be able to get 5x4, but not 6x4 - is this likely to be a problem?

You can get the form at KL. The pic of 6x4 can be obtained at the many shops nearby.
