Just visited the Thai embassy website in Washington DC and came across this. Not sure how many they actually give out, but there have been questions on these things from time to time, here at Thai Visa. No doubt, they probably are next to impossible to get!!

Rarer than hens teeth is the 3 yearer Samran.

Investigated this from the UK a while back and it is only issued by the Embassy there, not by the famous Hull consulate. Agreed it is a rare beast but far from impossible if you can jump through the right hoops. That said, if you can fulfill the criteria, then you probably have such a position or money that you don't need to worry about such a visa.
