If I subscribe to True TV and want to watch in 2 locations ie upstairs and upstairs do I need 2 boxes or just 1 with an extension

I realise I could only use 1 at a time

If you have no problems changing channels only at the box (downstairs?) then just run a RF cable via a three-way splitter to upstairs or use AV downstairs and RF upstairs (no splitter needed).

In this case you won't need a second box and save a lot of money.

This is our set-up.

I went for the 2nd box option in my previous condo, it's only an extra 437.63 Baht/month for each additional box on top of your normal subscription and you can use both at the same time watching different channels, the extra fee gets you a 2nd viewing card with the same channels as your 1st box.
