This may be a dumb question, but I want to return to Thailand on the last possible day to get the final 90 days on a non-imm O. The visa says "This visa must be utilized before..." and then written in pen "January 11, 2010".

So my question is, does that mean "on or before January 11, 2010" or does it really mean "before January 11, 2010?"

(And, also, can I assume there is no leniency, such as, a flight arrival time of 0100 in the am on January 12?)

"Before" means arrive on the 10th, or earlier.

Flying in? If the plane is actually reasonably late, reckon you'd be OK, like caused by a late departure from the scheduled time. But if it broke down, say in Hong Kong, and you came in one or two days later, you might not make it.
