Gladiator
I went to Chiang Mai Immigration this morning to apply for a Single Re-entry Permit. It may have been just a quirk, but after sumitting the required paperwork: A completed TM-8 form The correct fee A 2" photo Phtocopy of passport (main page, visa page and latest entry stamp) The Immigration Officer called me back and asked for a copy of the Arrival/Departure Card TM.6. Luckily it wasn't all that busy so quickly went out back and managed to get the photocopy. However when producing your own paperwork for the Re-entry application, you might also want to copy the TM.6 card, just in case.
