I usually take a photo, my Passport and the old Licence. Normally go to Talingchan which is a fairly relaxed Office. My only change is that I now wear glasses for mid distance but long and short distances are still OK. Any other Docs needed these days of which you are aware? Copy of your passport info page and current Non-O or Extension page. Proof of current address, I used my Yellow tambien ban, they were quite happy with it. And, yes, I realize the the address on the expiring license is good at banks, post offices, other government offices, but, TiT. You might try without this if the office is not too far away.
