Can I acquire the same forms from the US Consulate in CM (affidavit of income, residence) or do I need to get them at the Embassy in BKK? I would rather avoid the Embassy experience if possible.

They are available at the CM Consulate office.

You will have to go on line to make a appointment. Not sure of the cost under the new pricing system.

However there is a cost associated with the consulate notarizing your signature on any document. The last time I used the notarial services I believe the cost was 1,050 baht.

The notary fee is $30. Has been for years now. Affadavit of income is from the consulate but I think the affadavit of residence is back at Thai Immigration again.

There are now new fees as of last year.

4. Fee: $50.00 per notary seal. You may pay in U.S. dollars or the equivalent in Thai baht. Also, we accept most major credit cards.

Notary Fee now 1,500 thb as of this past monday.
