Can I acquire the same forms from the US Consulate in CM (affidavit of income, residence) or do I need to get them at the Embassy in BKK? I would rather avoid the Embassy experience if possible.

No cost for appointments, and alot more convenient and efficiant than the Embassy in BKK. :D

However there is a cost associated with the consulate notarizing your signature on any document. The last time I used the notarial services I believe the cost was 1,050 baht.

The notary fee is $30. Has been for years now. Affadavit of income is from the consulate but I think the affadavit of residence is back at Thai Immigration again.

There are now new fees as of last year.

4. Fee: $50.00 per notary seal. You may pay in U.S. dollars or the equivalent in Thai baht. Also, we accept most major credit cards.
