Fill out one copy of Form TM.47 at the office Proof of current residence, typically copy of utility bill, rental lease, etc. One copy passport ID page One copy original visa page One copy current permission to stay stamp page One copy latest immigration entry stamp page One copy airport landing card (stapled in passport) One copy may cover more than one of above items. Submitted the following docs: Passport Completed TM47 form 1 copy of passport ID page 1 copy of TM8 Departure Card 1 copy of current permission to stay stamp Proof of of current residence No probs.
